My mare is almost 16 hands, and she's got an unusually long neck, so I got her a XXL.
It fits her perfectly at the head, down the neck, and does not have issues staying put behind the withers. The wither strap is just a bit too long, but I was able to carefully tie a flattened knot in the strap twice and now it fits perfectly around her girth area.
I'm going to get straight to the point, this hood is one of the best I've used, and at a price point of about 30 dollars it's a steal. Especially if your horse is hard on their clothes, and you find yourself having to re-order the item.
Things best thing I have discovered is that it does not hug the neck tightly. There's room for the horse to bend and turn without having a second skin sucking it all in further.
There is no velcro around the muzzle, only a stretchy band. They eye holes are plenty big and the space around the ears is also plenty big. The nose band would only be an issue if your horse has a wide face. It has the potential to rub against the crest of the nose, which can cause hair loss in that area. If your horse's face tapers this is not an issue.
